About The Position
BAYADA Home Health Care is seeking a Medical Social Worker (MSW) to provide care to home health and hospice clients and their families in Sierra Vista, AZ. This is a Per Diem position where you will be an integral part of a multi-disciplinary health care team delivering skilled nursing and rehabilitative care to clients, enabling them to remain at home. The organization prioritizes delivering high-quality home health care with compassion, excellence, and reliability.
Requirements
- A minimum of a master's degree in social work
- At least two (2) years of community-based experience
- Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree
- Solid computer skills
- Ability to travel to cases, as assigned
Nice To Haves
- Home Health Care experience a plus
- Prior experience with electronic medical records (EMR) preferred
Responsibilities
- Assist team in understanding the social and emotional factors related to clients’ health problems
- Provide clinical social evaluations and plan appropriate interventions based on evaluation findings
- Identify community resources to address the client’s and family’s practical and environmental needs
- Act as an advocate for the client and family to navigate the patient through the community system
- Participate in the development of the total plan of care with the multidisciplinary team of BAYADA clinicians
- Develop and maintain contacts with public and private agencies as resources for the patient
- Educate and instruct patients, family members, or other patient representatives in community resources to promote the patient's health, safety, well-being, and independent living
- Accurately document observations, interventions, and evaluations of patient care and services provided utilizing a touchpad tablet
