About The Position

Assesses and identifies the client’s holistic health care needs/concerns as part of a family and community. Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care. Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols. Implements patient care plan activities based on treatment goals towards independent living for the patient/family. Serves as a resource for staff education & training, participates in case conferences as needed. This is a Per Diem LCSW position to support Palliative Care, Hospice, and Home Health.

Requirements

• Master’s degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure
• Current CPR card required.
• One or more years of experience as a master’s level social worker in a health care setting.
• A valid Drivers License and reliable car with insurance.
