The Medical Social Worker will complete psychosocial assessments for patients' post-hospital care, assisting them in coping with stress related to hospitalization, disability, and illness. This role involves collaborating with the Clinical Care Management team to assess and evaluate the appropriate level of care or environment setting for patients. The Social Worker will also serve as a resource to hospital staff and physicians regarding the emotional, social, and psychosocial aspects of a patient's illness, provide information and education on community resources for post-hospital care, and make referrals to community agencies as needed. Additionally, the position requires reporting suspected cases of abuse, neglect, or exploitation, serving as a liaison between the hospital, patients, and the Department of Social Services, and responding to consultative service requests after normal working hours.
