About The Position

This role involves assessing and identifying the client's holistic health care needs and concerns as part of a family and community. The Medical Social Worker will assess social/emotional factors related to the patient's illness, need for care, response to treatment, and adjustment to care. A plan of care will be developed under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols. Patient care plan activities will be implemented based on treatment goals towards independent living for the patient/family. The role also includes serving as a resource for staff education & training and participating in case conferences as needed. This is a Per Diem LCSW position to support Palliative Care, Hospice, and Home Health.

Requirements

• Master’s degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure- MSW or LCSW required
• Current CPR card required.
• One or more years of experience as a master’s level social worker in a health care setting.
• A valid Drivers License and reliable car with insurance.
