About The Position
PurposeCare is seeking a qualified, dependable, honest, and reliable Medical Social Worker (MSW) to support the needs of our clients. PurposeCare is a leading provider of home care and home health services in the Midwest, serving over 3,000 clients monthly. We focus on coordinating care to keep clients healthy and safe at home, ensuring our most vulnerable populations have the support needed to live full lives in their community. Requirements
- Master's Degree in Social Work is required
- Must possess a valid driver's license
Nice To Haves
- At least 1 year of social work experience in the healthcare field preferred
Responsibilities
- Assessment of psychological, emotional and/or economic factors affecting patient's limitations and potential for a lack of improvement
- Collaborates with the physician and other members of the health team to develop a Plan of Care
- Comply with all aspects of the company's compliance and HIPPA regulations
