The medical social worker is responsible for the implementation of standards of care for medical social work services. This role involves assessing the psychosocial status of patients, planning interventions, maintaining clinical records, and providing information and referral services. The social worker also serves as a liaison between patients, families, and community agencies, and collaborates with internal personnel to support patient care. Additionally, they will develop and maintain contracts with public and private agencies, participate in care planning and case conferences, and assist the healthcare team in understanding social and emotional factors affecting patient health. Discharge planning is also a key responsibility, along with other duties delegated by the Clinical Director/supervisor.
