About The Position
Andwell Health Partners is seeking a full-time Medical Social Worker (MSW) to join their Hospice/Home Health team. This role involves assessing clients' holistic health care needs, focusing on social and emotional factors related to their illness and treatment. The MSW will develop and implement care plans in collaboration with clients, families, and healthcare providers, aiming for independent living. The position also includes serving as a resource for staff education and training, and participating in case conferences. A Per Diem LCSW is also sought to support Palliative Care, Hospice, and Home Health services.
Requirements
- Master’s degree from a school of social work accredited by the council on social work education.
- Maine Board of Social Work Licensure- MSW or LCSW required
- Current CPR card required.
- One or more years of experience as a master’s level social worker in a health care setting.
- A valid Drivers License and reliable car with insurance.
Responsibilities
- Assesses and identifies the client’s holistic health care needs/concerns as part of a family and community.
- Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
- Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
- Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
- Serve as resource for staff education & training, participate in case conferences as needed.
