This role is responsible for completing psychosocial evaluations, providing counseling and support to patients and their social support systems, and assisting them in coping with the stresses of hospitalization, disability, and chronic/terminal illness. The Medical Social Worker collaborates with a multidisciplinary team to develop and implement care plans that address identified psychosocial needs. They also serve as a resource for hospital staff and physicians regarding the emotional and social aspects of patient care, intervene in crisis situations, and complete psychosocial assessments for post-hospital care. The position involves working with the Clinical Care Management team to determine appropriate levels of care and facilitate smooth transitions across the healthcare continuum. Additionally, the role requires maintaining knowledge of community resources, making referrals, reporting suspected abuse/neglect/exploitation, and documenting patient interactions in the medical record.
