Medical Social Worker, Hospice (Santa Clara County)
About The Position

Requirements
- Master of Social Work accredited by the Council on Social Work Education.
- Minimum of one year's experience in healthcare.
- Demonstrates good verbal and written communication, and organization skills.
- Possesses and maintains current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

- Experience in a hospice setting preferred.
Responsibilities
- Deliver varied social work services to Hospice patients and their families.
- Provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services, and community education, outreach, and referral.
- Perform initial psychosocial, emotional, spiritual and bereavement assessments and assist in the development and implementation of goal-directed IDG care plan.
- Conduct ongoing reassessments of patient/family needs and counseling as required.
- Provide short-term crisis intervention and individual or family counseling when indicated.
- Observe, assess and bring to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
- Develop and maintain contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
- Document comprehensive psychosocial, emotional and spiritual assessment clearly and concisely in a timely manner.
- Participate in in-service education programs.
- Participate in the development of the total plan of care and case conferences as required.
