About The Position

They offer home healthcare services for patients recovering from illness or injury, and also provide 24/7 hospice care for individuals with limited life expectancies or those no longer seeking aggressive medical treatment. The hospice care team includes a medical director/physician, medical social worker, skilled nurse, home health aide, spiritual counselor, and hospice volunteer. Requirements

  • Master of Social Work accredited by the Council on Social Work Education.
  • Minimum of one year's experience in healthcare.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Nice To Haves

  • Experience in a hospice setting preferred.

Responsibilities

  • Deliver varied social work services to Hospice patients and their families.
  • Provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services, and community education, outreach, and referral.
  • Perform initial psychosocial, emotional, spiritual and bereavement assessments and assist in the development and implementation of goal-directed IDG care plan.
  • Conduct ongoing reassessments of patient/family needs and counseling as required.
  • Provide short-term crisis intervention and individual or family counseling when indicated.
  • Observe, assess and bring to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
  • Develop and maintain contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
  • Document comprehensive psychosocial, emotional and spiritual assessment clearly and concisely in a timely manner.
  • Participate in in-service education programs.
  • Participate in the development of the total plan of care and case conferences as required.
