About The Position

NurseCore is seeking a Medical Social Worker for Home Health to serve clients in Santa Barbara and surrounding areas. This is a part-time position with flexible scheduling and daily pay options. The role involves assessing client needs, developing care plans, and working with clients and families to address social and medical needs. Accurate and timely documentation is essential, as is participation in in-service programs and case conferences.

Requirements

• Graduate of an accredited college with a Master of Social Work degree or equivalent
• Currently licensed in the State of California
• Completion of a minimum of one year of supervised work experience or the equivalent work experience in the field of psychology or sociology

Nice To Haves

• Experience in a home health care setting preferred

Responsibilities

• Completes assessments of client’s psychological, social and medical care needs and coping mechanisms
• Assesses the client’s home situation and financial resources available to meet the client’s medical and social needs
• Participates in the development and revisions to the client’s Plan of Care
• Works with the client and family members in meeting the client’s social and medical needs
• Maintains and submits accurate, timely, and complete documentation in accordance with agency policies
• Participates in in-service programs, case conferences, and discharge planning
