This role is responsible for completing psychosocial assessments for patients' post-hospital care, assisting patients in coping with stress related to hospitalization, disability, and illness. The Medical Social Worker collaborates with the Clinical Care Management team to assess and evaluate the appropriate level of care or environment setting for patients. They also serve as a resource to hospital staff and physicians regarding the emotional, social, and psychosocial aspects of a patient's illness and its impact on their support system. Additionally, this role involves providing information and education on community resources and post-hospital care options, making referrals to community agencies, reporting suspected cases of abuse/neglect/exploitation, and acting as a liaison between the hospital, patients, and the Department of Social Services. The position also requires responding to consultative service requests after normal working hours.
