This role provides medical social work support to community clinics. The Medical Social Worker will conduct psychosocial evaluations, provide counseling and support, and assist patients and their social support systems in coping with stress related to illness and hospitalization. They will collaborate with a multidisciplinary team to develop and implement care plans, serve as a resource for staff regarding psychosocial aspects of patient care, and intervene in crisis situations. The position also involves completing psychosocial assessments for post-hospital care, working with the Clinical Care Management team to determine appropriate care settings, and participating in multidisciplinary care planning meetings. Maintaining knowledge of community resources, making referrals, and reporting suspected abuse or neglect are key responsibilities. The role also focuses on identifying and resolving barriers to smooth transitions across the healthcare continuum and maintaining accurate case records.
