About The Position

This position works under general supervision of the Supervisor of Medical Social Services and provides medical case management and coordination of care services. In addition, this position supervises Medical Social Associates and Assistants and consults with care team in the development and organization of specific aspects of social services needed for positive health outcomes. The University of Illinois College of Medicine Peoria (UICOMP) educates 244 medical students and nearly 300 physician residents annually. It is one of four campuses that make up the nation’s largest public medical school. Requirements

  • Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
  • Based on institutional requirements for each position: Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker.

Responsibilities

  • Interviews consumers and completes on-going assessment of consumer needs.
  • Develops a Care Plan and provides linkage and referral as indicated by assessment, and consumer advocacy as appropriate.
  • Conducts home visits as necessary in order to fully assess client environment and care needs.
  • Maintains strict compliance with HIPAA, AIDS Confidentiality Act, and other UICOM-P and Positive Health Solutions policies and procedures.
  • Assists in writing Standard Operation Procedures related to Testing and Prevention services.
  • Co-chair of Testing committee and responsible for setting testing event schedules, and back-up tester schedules.
  • Monitors and maintains the standards and training for a specialized area of Social Service. This may include, but not limited to, Resources specific to geographical areas of our service region, Housing Services, Maternal Fetal Care, or the Department of Corrections.
  • Available to provide consultation, education and training to community agencies and well as inter-agency trainings.
  • Keeps accurate consumer records according to funding source guidelines and assists supervisor in data collection to complete monthly program reports.
  • Maintains awareness of and relationships with support services available to HIV+ and PWAs, including but not limited to various in-home professional and paraprofessional health care, nutrition, substance abuse treatment, housing, transportation and other available support services to ensure continuity of consumer care as appropriate.
  • Participates in department meetings, program meetings, and interagency meetings as requested by Medical Social Supervisor.
  • Perform other related duties and participate in special projects as assigned.
  • Performs work related to the lower level of this series.
