This role plays a vital part in coordinating medical appointments, maintaining health records, ensuring accurate medication documentation, and advocating for individuals while promoting dignity, independence, and person-centered care.   • High School Diploma or GED.
  • Valid driver's license with an acceptable driving record.
  • Basic computer skills, including Microsoft Office and web-based documentation systems.
  • Ability to complete required agency training.
  • CPR Certification (or ability to obtain).

Nice To Haves

  • Experience working in healthcare, human services, disability services, medical office administration, or direct support.
  • Experience with medication administration and healthcare documentation.

Responsibilities

  • Schedule, coordinate, confirm, and transport individuals to medical appointments.
  • Attend appointments as needed and communicate important information to healthcare providers and support teams.
  • Ensure medical documentation is completed accurately and maintained in compliance with agency standards.
  • Assist with medication tracking, medication orders, MAR updates, and medication distribution.
  • Pick up medications from pharmacies and deliver them to the appropriate locations.
  • Maintain current healthcare records, appointment schedules, and medical documentation.
  • Monitor and replenish medical and first aid supplies as needed.
  • Support individuals in understanding and participating in their healthcare needs.
  • Maintain confidentiality of health information in accordance with HIPAA guidelines.
  • Assist the Director of Medical Services with administrative and recordkeeping tasks.
  • Collaborate with nurses, residential teams, and healthcare providers to ensure quality care.
