Requirements

High school diploma or equivalent
Medical terminology/anatomy coursework
Must adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval.

Nice To Haves

Some college preferred
Previous experience working in an outpatient clinic or doctor’s office and/or Medical transcriptionist experience preferred

Responsibilities

Annotate dictated or written information for patient treatment following local, state, and federal guidelines for documentation in the electronic medical record.
Clearly record each patient’s symptoms, history, physical exam, and document the diagnosis as well as treatment plans.
Assist the Clinician with documentation of each patient’s medical chart during the patient’s visit.
Accompany the Clinician into the patient’s examination area to transcribe the history and physical exam with patient authorization.
Under the direction of the Clinician, transcribe patient orders, including laboratory tests, radiology tests, referrals, and retrieve necessary information for each patient’s visit.
Document any procedures performed by the Clinician within the electronic medical record.
Transcribe any consultations or discussions with family and/or the Clinician.
Complete the patient’s chart by transcribing results of any labs, x-rays, or other evaluations dictated by the Clinician.
Continuously check on the progress of data to ensure the patient’s workup is complete for the Clinician to make decisions regarding the patient’s prognosis.
List all proper diagnosis as well as any follow-up instructions as dictated by the Clinician.
Assist the Clinician and support staff with paperwork, telephone calls, faxing, and chart abstraction as assigned.
Process portal communications and either task to appropriate staff or get information from the Clinician to respond to the patient in a timely manner via portal, telephone, or other means as directed.
