About The Position

Gardner Health Services is currently recruiting to fill a Medical Referral Clerk/Case Manager position. This is a full-time, 40 hour/week position. The position is based out of the CompreCare Health Center located at 3030 Alum Rock Ave in San Jose, CA. Under the direct supervision of the Referral Department Supervisor, the Referral Clerk is responsible for processing referrals (OBGYN/Family Practice) and facilitates patient care follow-up, as it relates to patient referral. In addition, will be the direct contact between GFHN clinicians and referring clinicians as it relates to referrals. Interfaces with patients, medical staff, nursing staff, patient accounts referring/consulting agencies and support services in the resolution of issues relating to patient care in the referrals system. In addition, Referral Clerk will case manage any referral that requires additional attention with guidance from clinicians. As a team member in Patient Centered Medical Home, the Referral Clerk collaborates with clinic management, nursing, clinician staff, and clerical staff to maintain high standards of patient care with patient in the center.

Requirements

• High School graduate or equivalent
• Medical Assistant Certificate from an approved Medical Assistant program as required by the State of California
• CPR certified
• Bilingual skills in English-Spanish required
• Possession of a valid CA driver’s license

Responsibilities

• Processing referrals (OBGYN/Family Practice)
• Facilitating patient care follow-up as it relates to patient referral
