About The Position

The Medical Records Representative is responsible for sorting of daily incoming faxes; scanning patient’s consultations, laboratory reports, diagnostic reports and other documents into patient’s charts; fax out all documents requiring additional information to medical sites. Process incoming medical records from hospital portals. Process incoming medical record requests and requests for records from other facilities. Works collaboratively with external vendors and participates with legal requests for records. Requirements

  • Ability to communicate effectively with patients, co-workers, and the general public both in person, written and over the phone
  • One (1) to two (2) years of scanning and medical records experience and in addition one (1) to two (2) years of customer service experience
  • Developing basic knowledge of medical terminology
  • Accuracy and attention to detail
  • Ability to prioritize and perform multiple tasks simultaneously
  • Must possess strong organization skills
  • Be able to work independently and within a team
  • MS Office: Particularly Excel, Word, and Outlook skill
  • Computerized database experience a plus
  • Office skills: typing, faxing, scanning and telephone
  • High school diploma or GED from an accredited institution

  • Willingness to be cross-trained and learn new skills and responsibilities
  • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations

Responsibilities

  • Responsible for scanning incoming documents into patient’s charts accurately
  • Sort incoming faxes and correspondence
  • Research and verify within patient’s chart
  • Ability to perform work at established production rates
  • Maintain effective communication with office staff and providers in all the health centers as needed
  • Demonstrate respectful and professional behavior that supports a team-oriented work environment
  • Attend staff meetings, in-service meetings, or other required meetings
  • Maintain satisfactory attendance, is punctual and works full scheduled shift
  • Meeting minimum standards of metric expectations
  • Other duties as requested or assigned
