This position supports participants receiving home and community-based services by reviewing incidents, service plans, and related documentation.
Requirements
- Three years of professional experience in the field of medical assistance, health care services, or human services; or An equivalent combination of experience and training.
- Active authorization to practice as a Registered Nurse in Pennsylvania. If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
- Active motor vehicle license.
- Meet the PA residency requirement.
- Able to perform essential job functions.
Responsibilities
- Assess reportable incidents and examine service plans for accuracy and appropriate service delivery
- Collaborate with service coordinators, providers, and agencies to address critical incidents and uphold participant welfare
- Analyze medical documentation to determine service suitability and identify needed referrals
- Review incident reports to identify patterns that influence participant service needs
- Assist with monitoring existing programs and developing new home and community-based service initiatives
