This position supports participants receiving home and community-based services by reviewing incidents, service plans, and related documentation.

Requirements

  • Three years of professional experience in the field of medical assistance, health care services, or human services; or An equivalent combination of experience and training.
  • Active authorization to practice as a Registered Nurse in Pennsylvania. If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
  • Active motor vehicle license.
  • Meet the PA residency requirement.
  • Able to perform essential job functions.

Responsibilities

  • Assess reportable incidents and examine service plans for accuracy and appropriate service delivery
  • Collaborate with service coordinators, providers, and agencies to address critical incidents and uphold participant welfare
  • Analyze medical documentation to determine service suitability and identify needed referrals
  • Review incident reports to identify patterns that influence participant service needs
  • Assist with monitoring existing programs and developing new home and community-based service initiatives
