About The Position

The Medical Records Clerk is responsible for receiving and processing patient records following discharge. This role involves going to various units to pick up discharge records, depositing them into HIM processing areas, and ensuring all paperwork is received and complete. The clerk will accurately analyze incomplete records for compliance with regulatory standards and Medical Staff Bylaws, Rules & Regulations, and consistently meet performance expectations for record analysis. Responsibilities include adding/resolving deficiencies in the computer system, retrieving records from retention storage, maintaining the Master Patient Index, and meeting established deadlines. The role also requires prepping and scanning paper patient information into the electronic health record efficiently and accurately to facilitate timely coding. Participation in Performance Improvement and Quality Control Activities, including data gathering, validation, and report preparation, is expected. The clerk will demonstrate teamwork, initiative, and courteous behavior. Additional duties include assisting with other clerical tasks in the department, aiding physicians with incomplete records, releasing patient information according to rules and regulations, documenting ROI, and scanning confirmations of continuity of care requests. The position requires basic computer knowledge, the ability to handle interruptions, and age-specific patient needs awareness. Information management, risk management, quality management, and safety are key aspects of the role.

Responsibilities

  • Receive and process patient records following discharge.
  • Go to various units, pick up prior day’s discharges, and deposit records into respective HIM processing areas.
  • Ensure all prior day discharges have been received and retrieve any missing paperwork.
  • Accurately analyze incomplete records for compliance with regulatory standards and Medical Staff Bylaws, Rules & Regulations.
  • Consistently meet performance expectations for record analysis.
  • Add/resolve deficiencies within the computer system accurately.
  • Retrieve records that are in retention storage.
  • Maintenance of the Master Patient Index.
  • Meet established deadlines.
  • Prep and scan paper patient information into electronic health record in a timely, efficient, accurate manner.
  • Participate in Performance Improvement and Quality Control Activities by data gathering, validation of data, and/or preparing accurate reports.
  • Consistently demonstrate behavior that reflects working as a team is more important than self-interest.
