About The Position

This position is responsible for managing the release of information process for medical records, ensuring compliance with HIPAA and organizational policies. The role involves tracking requests, verifying authorizations, maintaining records in the EMR system, and reporting on outstanding deficiencies. The Medical Record Clerk will also assist with scanning and archiving, and answer phones as needed. Responsibilities

  • Adhere to all policies of the organization, including job description, certification/licensure requirements, mission statement, HIPAA, confidentiality standard and patient’s rights.
  • Stay up to date with organization communication and directive.
  • Track and monitor the entire release of information process until the requested medical records are received including faxing of medical requests to providers, hospitals, and other healthcare facilities.
  • Record requests completed timely as stated in Medical Records Procedure.
  • Reviews the quality and legitimacy of each request and determines if information can be released.
  • Checks for properly executed authorization to release medical information on designated patients.
  • Maintains working knowledge of state statutes on authorization requirements and related fees for copies and/or viewing.
  • Enters and logs all requests into the release of patient information software system within the electronic health record and/or excel.
  • Maintains thorough knowledge of “Release of Information” policies and procedures guide and uses keen judgment in releasing information to a wide variety of requestors, including physicians, healthcare providers, attorney, patients, state agencies, insurance officers, etc.
  • Review patient medical records to identify incomplete, missing or inaccurate information.
  • Enter, update, and track deficiencies in the electronic medical record (EMR) system.
  • Provide monthly report of outstanding chart deficiency to supervisor.
  • Meet established deadlines and productivity standards for chart completion and deficiency resolution.
  • Assist with any scanning and archiving records.
  • Answers phone as needed.
  • Other duties as assigned.
