About The Position

The Care Navigator provides a welcoming experience for members and their families, guiding members through their visit and helping ensure their care needs are met. Requirements

• Excellent customer service skills and a positive, welcoming demeanor.
• Passion for providing a quality experience for senior members.
• Ability to work effectively with diverse populations, including staff, providers, members, family members, insurance carriers, vendors, and the public.
• Working knowledge of medical terminology, insurance, and/or electronic medical record systems.
• Strong organization and attention to detail.
• Strong written and verbal communication skills.
• Proficiency with PC skills, including Microsoft Office Suite.
• Ability to maintain professionalism and flexibility in a changing work environment.
• At least one year of work experience in a clinical setting or similar.

Nice To Haves

• Associate degree preferred, or equivalent experience.
• Experience working with EMR systems.
• Fluency in Spanish or other languages spoken in the communities served, where necessary.

Responsibilities

• Welcome members and guide them through a clear, seamless check-in process.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for the multidisciplinary care team.
• Help members complete paperwork using electronic kiosks when needed.
• Request medical records and upload documentation to the electronic medical records system.
• Answer questions from prospective and established members and their adult children.
• Answer incoming phone calls, take detailed messages, and route pertinent information to the appropriate person.
• Assist with center events as needed.
