About The Position

Eterna Primary Care is looking for a Medical Practitioner (NP/PA) who is passionate about geriatric and post-acute care to join our mission-driven team. You will bridge the gap between acute crisis and long-term wellness, ensuring that every patient receives high-quality, patient-centered care designed to reduce hospital readmissions and improve quality of life.

Requirements

A Master’s or Doctorate from an accredited program (NP or PA) with a current state license, Board Certification, and an active DEA.
A background in Geriatrics, Post-Acute, or SNF settings is strongly preferred. You understand the unique rhythm of the post-acute environment.
You are independent yet collaborative, a master of time management, and a compassionate communicator who treats every patient with profound respect.

Responsibilities

Perform comprehensive assessments and manage acute and chronic conditions in complex, medically fragile populations.
Utilize telehealth and evidence-based strategies to enhance patient outcomes and improve the overall care experience.
Act as the vital liaison between hospitals, families, and facility leadership. You’ll be the "go-to" expert for nursing staff and therapy teams.
Champion quality metrics and cost-effective medicine that prioritizes the patient’s goals over everything else.
