Requirements
- Unrestricted professional license.
- Ability to practice independently and complete accurate, timely documentation.
Nice To Haves
- Experience in geriatrics, preferred.
- Experience in post-acute care, preferred.
Responsibilities
- Primary responsibility for primary care services, acute care services, routine monitoring of chronic conditions.
- Implementing best practices to avoid hospitalizations and emergency department visits and maximize functional improvements and quality of life.
- Collaborate and provide clinical support to nurse practitioners, typically one per facility, empowering them with guidance and sharing medical expertise.
