Requirements

• Must be a strong Medical Discharge Planner.
• Must be able to work full-time.

Nice To Haves

• Experience in correctional healthcare.
• Familiarity with electronic operating systems.

Responsibilities

• Conduct rounds with physicians and mental health staff to meet with patients to discuss placement options, organize records for transition, and provide advice regarding community assistance organizations and transportation options.
• Involve patients in their discharge planning process.
• Assist patients in identifying a support network and provide instructions for future care and follow-up.
• Assist patients with the completion of all clinical and financial documentation required for social service organizations.
• Coordinate with healthcare and security staff regarding patients’ release dates.
• Serve as a liaison for all referrals for involuntary examinations or crisis stabilization.
• Serve as a liaison for all inpatient care management.
