This Medical Director – Health Care Delivery, S&D role will put you at the leading edge of the value-based care movement by being a part of our Central Care Model Development team. The Medical Director – Health Care Delivery, S&D will be responsible for co-leading, with Directors and Vice Presidents from the Population Health and Central Services Departments, a range of clinical and strategic initiatives. The Population Health program is led by the Chief Population Health and Chief Clinical Officers and focuses on the design and implementation of the Care Model. The Product team which designs, builds and implements software tools to support care team workflows and meet their information needs is a part of the Population Health team and therefore technology work will be part of the scope of the team and position. Although most of the care delivery is organized through its Divisions and Regions, there are also centralized clinical efforts which most often reside within the Population Health or Centralized Services Teams. The Medical Director – Health Care Delivery, S&D, APP will provide clinical leadership and expertise to a portfolio of programs and planning initiatives within this domain. Core responsibilities will include: Acting as the primary clinical authority on the most common chronic and acute conditions affecting the Oak Street Health patient population. They will partner closely with operational, product, and field teams to develop and implement scalable strategies for screening and accurately diagnosing these conditions. Planning and building comprehensive educational materials and clinical resources for the monthly Diagnosis Excellence and Clinical Guidelines Educational Series (DECGES). Additionally, they will lead and present at the monthly Diagnosis Excellence sessions, effectively delivering insights and updates to all OSH providers and scribes. Performing provider level chart reviews to align with screening and diagnosis excellence needs, and synthesize recommendations based on these chart reviews for other Population Health leaders. Working closely with Screening and Diagnosis Excellence leadership Additional responsibilities may include: Clinical Teams within our call center, referrals and medical records departments Liaison with external contracted disease management companies Specialty Services embedded in the primary care centers Pharmacy and Medication Management activities Core Responsibilities: In addition to clinical duties caring for patients as a primary care provider 2 days per week in one of our centers, the Medical Director is responsible for the following leadership and administrative duties: Population Health program clinical leadership: This physician or Advanced Practice Provider will be the clinical leader for a set of programs within the Population Health and Centralized Services Teams. As the clinical leader for the programs, it will be the Medical Directors responsibility to assure the clinical services are high quality, patient centered and address broader goals. Depending on the program design, the Medical Director may be expected to provide direct clinical supervision to RN’s, NP’s, physicians, and/or other clinical or support staff involved in delivering care through these programs. Liaison to other clinical and operational leaders: Patients receiving care through these clinical programs will, in most cases, also be receiving primary care through our network of primary care centers. Therefore, it is critical that all programs continuously communicate with the patient’s primary care team members to assure patient, rather than service line, centered care. The Medical Director will be expected to consistently seek input from Medical Directors and other clinical leaders supporting and working within our primary care centers. The Medical Director will also be responsible for informing all stakeholders regarding program activities and efforts to assure excellent communication and collaboration. Organizational Strategy and Collaboration: The Medical Director will participate in organization wide planning and strategy including helping vet new opportunities, plan staffing, and hiring staff. The Medical Director will also collaborate closely with Population Health, Central Services, and Divisional / Regional leaders in these efforts. Additionally, the Medical Director may be asked to participate or lead workgroups or committees. Drive Culture: We foster a culture of innovation and open dialogue, and we work with urgency.
