About The Position

Theoria Medical is seeking a Medical Director and Attending Physician to serve local skilled nursing facilities within a close-knit, mission-driven care community. This role involves providing high-quality, patient-centered primary care, diagnosing and treating acute and chronic conditions, and participating in various quality initiatives. The position offers flexible scheduling, autonomy, and support through intuitive technology and a collaborative clinical team, allowing physicians to focus on delivering exceptional care and building meaningful relationships with patients over time.

Requirements

Physicians (MD/DO) must hold an active U.S. medical license.
Must be board-certified or board-eligible in an appropriate specialty.
Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.

Responsibilities

Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
