The Los Angeles County Department of Homeless Services and Housing (HSH) is consolidating its countywide response to homelessness. The Homeless Prevention Unit (HPU) is a collaborative effort between HSH, California Policy Lab (CPL), and the Department of Mental Health (DMH). The HPU proactively identifies individuals at risk of homelessness through predictive analytics and provides early interventions. The HPU is expanding to serve young adults aged 18-24 in need of proactive homelessness prevention resources. This role involves significant interaction with various County departments to navigate service systems and connect individuals to services for housing stability. The ideal candidate will be knowledgeable in programs and services for young people at risk of or experiencing homelessness and flexible to meet changing needs. The Medical Case Worker II (Outreach Coordinator) will perform full-time proactive outreach for the HPU, reporting to the Outreach Program Manager. This position will outreach to and engage prospective clients identified as high risk of homelessness, gather consents, and facilitate warm hand-offs to HPU Case Managers.
