About The Position

Hamilton Health Center is seeking a Medical Case Manager to provide specialized case management services to individuals living with HIV through our HOPE program under the Ryan White Part B program. This role plays an important part in improving health outcomes by helping clients access appropriate medical care and support services, promoting treatment adherence, and supporting continuity of care. The Medical Case Manager will work collaboratively with an interdisciplinary team to provide patient-centered services while ensuring accurate documentation and compliance with applicable program, funding, and regulatory requirements.

Requirements

  • Knowledge of case management principles and functions within a healthcare setting
  • Proficiency in data entry with the ability to learn and utilize program-specific systems and databases
  • Strong interpersonal and communication skills with the ability to engage individuals from diverse backgrounds
  • Demonstrated cultural humility and understanding of diverse populations and health disparities
  • Strong organizational skills with attention to detail and compliance requirements
  • Ability to work collaboratively with clinical teams, leadership, and community partners
  • Valid driver’s license with the ability to travel throughout the service area

Nice To Haves

  • Bachelor’s degree in Social Work, Psychology, Sociology, or a related field
  • Two (2) years of experience in case management or a comparable social services role
  • Experience working with individuals living with HIV and conducting rapid HIV testing in community settings
  • Knowledge of community resources in Harrisburg and surrounding areas
  • Bilingual (Spanish)

Responsibilities

  • Provide specialized medical case management services to individuals living with HIV
  • Facilitate timely and coordinated access to appropriate medical care and support services to promote continuity of care
  • Conduct treatment adherence activities to assess client readiness for and adherence to HIV treatment
  • Maintain compliance with documentation, reporting, and client contact requirements established by program funding organizations and regulatory bodies
  • Enter required documentation accurately and efficiently into appropriate electronic systems
  • Collaborate with interdisciplinary teams to support a patient-centered approach to improving health outcomes
  • Conduct rapid HIV testing in both office and community-based settings
  • Participate in outreach activities, community events, and patient engagement initiatives
  • Complete and maintain required continuing education in accordance with program regulations
