About The Position
The Medical Case Manager ensures that all PATH adult program patients are referred to appropriate health and psychosocial services, as well as for entitlements.
Requirements
- High School or GED
- At least two (2) years of experience in HIV treatment, medical case management and/or health education
- Must have strong written and oral communication skills.
Nice To Haves
- Associate or Bachelor’s Degree
- Prior experience using an Electronic Medical Record
- Bilingual English/Spanish or English/Creole
