As a vital member of the Health and Housing Transition (HHoT) Team at Fourth Street Clinic, the Medical Case Manager plays a key role in transforming lives for those within Magnolia Apartments. This dynamic position involves assisting patients and families in navigating the maze of medical, social, educational, and other supportive services necessary for their well-being. Located in a permanent supportive housing program for individuals and families with a history of chronic homelessness, you'll be at the forefront of making meaningful connections. With collaboration as your superpower, you'll work closely with tenants and community partners like The Road Home, managing referrals and follow-ups for healthcare services, insurance eligibility, transportation, and addressing social determinants of health. Requirements

  • High school diploma or equivalent
  • Social service or community work experience
  • Obtain Utah case management certificate within 90 days of hire.
  • Must be able to pass a background check.

Nice To Haves

  • Bachelor's degree in social work social services or similar degree
  • Social Service Worker (SSW) Utah license
  • Medical case management or care coordination experience
  • Experience working with homeless or vulnerable populations.
  • Ability to communicate in Spanish is desirable.
