The Medical Case Manager is responsible for providing those living with HIV a comprehensive continuum of services through critical linkages with community resources. This includes the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV treatments. This position entails an assessment of service needs; development of a comprehensive, individualized service care plan; coordination of services required to implement the plan; client monitoring; and re-evaluation and adaptation of the plan over the life of the client. Contacts with clients will be face-to-face at medical, home, office, and hospital visits. This position reports to the Manager of Care Services. This is a HYBRID position. The Medical Case Manager applies the principles of the organization’s signature service delivery approach, Primary Client Model (PCM™), by promoting a positive employee/client-focused experience that is appreciative of the unique strengths, values and needs that each individual brings; and by using data to inform and drive decision making.
