About The Position

Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. Our mission is to provide quality, affordable health care for all including those without the ability to pay. We currently have health centers in Des Plaines, DeKalb, Elgin, Wheeling, Hanover Park, Streamwood, Sycamore, McHenry, Palatine and Franklin Park. We are financially and programmatically stable and are continuing to grow. We are Joint Commission Accredited and are PCMH Certified. This is a great job opportunity for an experienced Medical Case Manager (HIV/AIDS). In this position, you’ll be able to contribute to your community and make an impact in a patient’s life.

Requirements

  • Bachelor’s degree in related field is required.
  • Bachelor's degree in social work (BSW), human services, social science, or health education field.
  • Active and current Basic Life Support certification (BLS) is required within 30 days of start date.
  • Excellent writing skills.
  • Computer literate and has ability to utilize various software, word processing and spreadsheet programs.

Nice To Haves

  • Bilingual in Spanish is strongly preferred but not required.
  • A minimum 1-year experience in Case Management is highly preferred but not required.

Responsibilities

  • Compete initial eligibility assessment.
  • Development of comprehensive, individualized care plan jointly with the client/patient that includes short- and long-term goals focused on attaining, maintaining, and achieving positive health outcomes.
  • Facilitate timely and coordinated access to medically appropriate levels of health and support services and continuity of care.
  • Provide continuous client/patient monitoring to assess the efficacy of the individualized care plan.
  • Coordinate the individualized care plan with client’s/patient’s primary care provider.
  • Refer and link client/patient to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency.
  • Monitor the client’s follow through with these services.
  • Enter and utilize the client/patient level database for the tracking and reporting of all services provided.
  • Follow the Standard Operating Procedures of the AIDS Foundation of Chicago’s Northeastern Illinois HIV/AIDS Case Management Collaborative.
  • Engage in all offered training, consultations and supervisions made available.
  • Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.
  • Perform other duties as assigned.
