About The Position

We are seeking a highly organized, compassionate, and dedicated Medical Care Coordinator to join our team. In this vital role, you will bridge the gap between our clients and the essential medical services they need. By managing appointments, securing benefits, and ensuring medication compliance, you will provide the stability and support our clients need to successfully navigate their recovery journey.

Requirements

  • High School Diploma required
  • 1+ years of Care Coordination
  • 2+ years in Behavioral Health
  • Strong computer literacy with hands-on comfort using Electronic Medical Record (EMR) systems.
  • Exceptional communication, proactive problem-solving, and a deep ability to work effectively with diverse populations.

Nice To Haves

  • Bachelor’s degree preferred.
  • Experience working with individuals navigating substance use disorders is highly preferred.

Responsibilities

  • Assist clients in accessing critical medical, dental, vision, housing, financial, and legal advocacy services.
  • Support clients in obtaining, tracking, or reactivating vital resources like Medicaid and disability benefits.
  • Oversee timely medication distribution, conduct daily compliance checks, and log updates accurately in the EMR system.
  • Monitor new prescriptions to guarantee seamless documentation and uninterrupted client care.
  • Complete and upload all incidental funds paperwork with strict accuracy and adherence to agency policy.
  • Collaborate closely with counselors to build thorough, safe, and effective discharge plans.
