The Medical Assistant for Clinical Risk Assessments will work alongside the Advanced Practice Clinician (APC) or other clinicians helping conduct clinical visits through telehealth and in-home visits with patients identified in the health system with high medical risk factors and suspected care gaps. The Medical Assistant responsibilities include patient flow management such as vital signs, assessment questionnaires, phlebotomy, injections and other various diagnostic procedures. Acts as liaison to clinicians to ensure patient satisfaction. Maintains and orders supplies for their area as needed. As schedule allows, the Medical Assistant may also participate in other service types within Jefferson Primary Care, including Supplemental Access Visits and/or coverage of other clinical services both telehealth and in-home.
