The Medical Assistant for Clinical Risk Assessments will work alongside the Advanced Practice Clinician (APC) or other clinicians helping conduct clinical visits through telehealth and in-home visits with patients identified in the health system with high medical risk factors and suspected care gaps. The Medical Assistant responsibilities include patient flow management such as vital signs, assessment questionnaires, phlebotomy, injections and other various diagnostic procedures. Acts as liaison to clinicians to ensure patient satisfaction. Maintains and orders supplies for their area as needed. As schedule allows, the Medical Assistant may also participate in other service types within Jefferson Primary Care, including Supplemental Access Visits and/or coverage of other clinical services both telehealth and in-home. • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson • Supports patients via telehealth and in-home and ensures proper identification. • Performs and documents vital signs, clinical care quality measures and medical history information. • Remains proficient and knowledgeable regarding the use and documentation requirements within information technology systems within established scope of practice. • Assists APC with examinations, various diagnostic procedures, and may perform venipuncture/injections, using appropriate skills to provide quality care to the patients. • Adheres to HIPAA privacy and security requirements. • Orders clinical supplies to maintain inventory and to facilitate smooth operations of the office
