About The Position

Get out of the clinic routine and deliver preventive care directly in patients’ homes across Oregon. This field-based role involves visiting patients in their homes to complete Medicare Annual Wellness Visits and Well-Child Visits, focusing on prevention and early intervention. You will build stronger patient relationships without rushed visits, chart using modern EMR tools, and work independently with clinical team support.

Requirements

• 1+ year as a Medical Assistant
• CMA, RMA, or equivalent certification
• 6+ months in primary care or internal medicine
• Comfortable with state travel
• Reliable vehicle and valid driver’s license
• Strong communication and documentation skills

Nice To Haves

• Medicare Annual Wellness Visit experience
• Well-Child Visit experience
• Home health or community-based care experience
