About The Position

Medical Assistants are core members of the Daymark care team, supporting integrated, whole-person care for our patients. In this role, you will coordinate social and logistical aspects of care, spending time connecting with patients and caregivers by phone, video, and occasionally in the home. You will help patients navigate community-based and social services, ensure follow-up appointments are scheduled, and support seamless coordination between Daymark and external providers.

Requirements

  • At least 3 years of experience as a Medical Assistant, CNA, EMT, or in a similar clinical or care coordination setting.
  • Comfortable driving to visit patients in the field.
  • Comfortable using Slack, Zoom, the Google Suite, and EHR systems to ensure accurate appointment scheduling and management and support patients virtually.
  • Bias to action: You don’t wait for someone to tell you what to do. You know how to identify and prioritize your initiative’s needs and you do what it takes to ensure that urgent and important needs are acted on immediately.
  • Comfort with change and ambiguity: You may not always have direction in your work, and that’s OK, because you’re comfortable figuring out what needs to be done and a path forward.
  • Strong relational skills. You have an uncanny ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
  • Organized & Communicative. You are incredibly organized and sweat the details. You keep your team in the loop with what you’re working on and the challenges you’re facing and are proficient in collecting patient clinical and demographic data and documenting appropriately in a timely manner.
  • Comfortable working out of our office in Providence, Rhode Island 5 days of the week, and visiting patient's homes whenever needed.
  • Basic Life Support (BLS) certification.
  • Compliance with Daymark's Employee Vaccination and Testing policy.
  • Proof of a valid driver's license.

Nice To Haves

  • Oncology experience is a bonus.
  • Bilingual is a plus but is not required.

Responsibilities

  • Conduct structured social needs assessments (SDOH) and connect patients with community resources, which may include transportation, financial assistance, food, medical equipment, or emotional support programs.
  • Support development of patients’ care plans and participate in case conferences to ensure quality care delivery.
  • Serve as the primary point of contact for virtual visit readiness and basic technology troubleshooting.
  • Coordinate and schedule care for patients, including scheduling follow-up visits and facilitating referrals.
  • Own structured chart preparation to ensure providers are visit-ready, including accurate contact, insurance, and care team information.
  • Track completion of our Symptom Survey, complete all appropriate follow ups, and escalate clinical concerns as needed.
  • Document social interventions, tasks, and care coordination activities in the electronic health record in a timely and standardized format.
