About The Position
Oversees CCM and RPM patient populations by monitoring vital signs and health data, identifying trends or abnormalities, and escalating concerns to providers as needed. Ensures accurate documentation, supports care plan execution, and maintains ongoing communication with patients and care teams to promote quality care and prevent avoidable utilization.
