About The Position

Works alongside nurses and BHMPs to assist in the flow and non-technical assessment of clients in an integrated behavioral health outpatient clinic. The role involves coordinating care and services to enhance the physical and behavioral health wellness of persons served within a trauma-informed care framework. This includes facilitating communication between medical and behavioral health practitioners, assisting with scheduling and compliance with physical health interventions, integrating treatment for both physical and behavioral health needs, and educating persons/families on the interplay of these issues on overall wellness. The position requires the ability to perform essential functions safely, with or without reasonable accommodation.

Requirements

• High School or equivalent required.
• Must hold an MA certificate.
• Must have knowledge of vital signs and the concept of electronic charting.
• Must have the ability to automate and digitalize medical equipment.
• Teamwork is crucial.
• Must be able to arrange transportation to work among work sites, if needed.

Nice To Haves

• Bilingual Preferred.
• Possession of first aid and CPR certification.
• Physical Exam and T.B. Test
• Fingerprint Clearance

Responsibilities

• Take vital signs under the direct supervision of an LPN, RN, PCP, or BHMP and recognize abnormal vital signs, and report these to nursing and BHMPs.
• Organize and dispense laboratory slips, medication requests, and pharmacy communications.
• Coordinate all care and services required to enhance and facilitate the optimal physical and behavioral health wellness of persons served within a trauma-informed care framework.
• Facilitate communication between medical and behavioral health practitioners/systems.
• Assist with scheduling of and compliance with preventative and specialty physical health interventions.
• Integrate treatment interventions to address both the physical and behavioral health needs of the person/family concurrently.
• Assist persons/families served with education and resources outlining the interplay of physical and behavioral health issues on overall wellness and quality of life.
• Administer injections (may be required).
• Draw laboratory samples and facilitate follow-up medical treatment.
• Perform related duties as assigned.
