Works alongside nurses and BHMPs to assist in the flow and non-technical assessment of clients in an integrated behavioral health outpatient clinic. The role involves coordinating care and services to enhance the physical and behavioral health wellness of persons served within a trauma-informed care framework. This includes facilitating communication between medical and behavioral health practitioners, assisting with scheduling and compliance with physical health interventions, integrating treatment for both physical and behavioral health needs, and educating persons/families on the interplay of these issues on overall wellness. The position requires the ability to perform essential functions safely, with or without reasonable accommodation.
