About The Position
The Medical Assistant supports the Transitional Care Clinic by assisting with patient care, rooming patients, obtaining vital signs, coordinating follow-up care, managing supplies, and supporting patients during their transition from hospital to primary care to promote recovery and prevent readmissions.
Requirements
- Certifications: Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
- Education: H.S. Diploma/GED (Required)
Responsibilities
- Performs routine direct patient care duties to assist medical and nursing staff with patient examinations and treatments
- Maintains patient care environment, equipment, and supplies to ensure patient privacy and safety
- Rooms patients and prepares them for provider examinations
- Obtains and records vital signs
- Collects and documents patient data in N2Q0D
- Stocks and organizes exam rooms and clinical supplies
- Conducts patient reminder calls for scheduled appointments
- Assists with setup and preparation for minor office procedures
- Supports efficient patient flow and clinic operations
- Adheres to established safety, infection control, and patient care protocols
