About The Position

Residents in senior living communities deserve care that's coordinated, consistent, and truly follows through, and that's exactly what you'll make possible. As a Medical Assistant Coordinator at Curana Health, you'll be the operational backbone for a dedicated panel of Senior Living providers, keeping everything running smoothly so providers can focus on what they do best: caring for patients. This isn't a background role. You'll be embedded in the care team, working on-site at senior living communities, building real relationships with residents, families, facility staff, and providers, and making a meaningful difference in how care gets delivered every day.

Requirements

  • High School Diploma or GED required
  • Active Medical Assistant credential (CMA, RMA, CCMA, or equivalent) preferred
  • 2+ years of experience in medical assisting, care coordination, senior living, primary care, post-acute, or a related healthcare setting
  • Comfort navigating EHR systems and digital tools
  • Strong organizational instincts and a follow-through mindset
  • Clear, confident communication with patients, families, facility partners, and care teams
  • An understanding of Medical Assistant scope and the good judgment to escalate when something needs clinical attention
  • Ease working across multiple providers, facilities, and workflows without losing detail
  • Willingness to travel within your assigned market to support your communities on-site

Nice To Haves

  • Associate's degree or healthcare certification
  • Experience with gEHRimed, Innovaccer, or PEARL

Responsibilities

  • Manage daily workflows, coordinate new visits, follow-ups, and acute care, and handle day-of reprioritization.
  • Prep charts before each visit, review gaps, chase down records, and complete quality checks.
  • Coordinate orders, referrals, diagnostics, labs, DME, home health, hospice, and outside providers, tracking every handoff to completion and escalating as needed.
  • Maintain the panel by accurately and consistently updating admissions, discharges, consent, hospice status, DNR changes, and care-team updates.
  • Communicate professionally with patients, families, and facility partners through approved channels.
  • Support CCM, APCM, and quality-gap closure activities, helping to improve care plan compliance and program engagement.
