About The Position

Working with a team, Medical Appointment Coordinators (MAC’s) are responsible for the coordination of all medical appointments and all necessary communication and documentation as needed to link individuals who live in residential sites to proper medical care. Responsible for providing direct support as needed. Arc GLOW is a parent and family driven organization that empowers and supports people of all ages with a broad spectrum of emotional, intellectual and developmental disabilities.

Requirements

High School Diploma, or equivalent required.
Valid NYS driver's license with a safe driving record
Excellent communication skills.
Has a willingness to learn new skills (we provide paid training).
Possesses computer skills.

Responsibilities

Assisting people supported in receiving needed/desired medical services and communicating and documenting pertinent information as per program policy (i.e., medication administration, transportation to appointments).
Ensuring an adequate amount of medication is on site for each individual at all times.
Providing communication to residential site staff to ensure physician and clinical orders are understood and followed.
Making recommendations to the CSRN and/or Health Care provider to maintain and/or improve the overall health and wellness of individuals supported.
Completing required paperwork on daily / as needed basis.
Providing direct support as needed.
Provide transportation as needed with agency vehicles.
Use various software platforms for specific job functions.
