About The Position

The Health Services Clinic at Rising Ground is a pediatric primary care clinic for children and adolescents in the Foster Care, Residential, and Justice for Youth and Families Programs. The Medicaid Managed Care Liaison is responsible for working together with the Health Services clinical and administrative teams to ensure the delivery of high quality, and continuity of care. This includes frequent interactions with clients and their families, service providers, care coordinators, and insurance companies. The Health Services Clinic is in Brooklyn, near a major subway hub.

Requirements

  • High School Diploma minimum requirement
  • Minimum of two years’ experience in a healthcare setting
  • Excellent computer skills, proficiency in Microsoft Office (Word and Excel), familiarity with electronic health records
  • Bilingual Spanish preferred
  • Authorized to work in the U.S.
  • Ability to work in-person in New York City (NYC)
  • Ability to travel to other Rising Ground Sites (if required)

Responsibilities

  • Participates in interviewing and hiring of Family Childcare Providers.
  • Answer phones at the Clinic.
  • Greet clients/confirm registration of clients
  • Enter and track demographics information in Electronic Medical Record
  • Opening clinic mail, answering email from the medicaloncall mailbox
  • Track subspecialty referral appointments, obtain documentation of subspecialty appointments, assist with additional medical records requests as needed
  • Support the coordination of healthcare and behavioral health services for children and youth.
  • Coordinate and assist with scheduling medical appointments, psychiatric evaluations, and medication management appointments.
  • Retrieve medical, psychiatric, and behavioral health documentation from outside and community-based providers.
  • Communicate and follow up with internal staff, outside providers, and other stakeholders to obtain required documentation and support continuity of care.
  • Maintain confidentiality when handling sensitive clients, medical, psychiatric, and behavioral health information.
  • Provide assistance to coworkers and contribute to team projects and departmental responsibilities as needed
  • Participate in quarterly Medicaid Managed Care meetings
  • Prepare forms and letters as requested
  • Upload documents in the Electronic Medical Record
  • CNNX note entries ACS system
  • Other related duties as assigned by supervisor.
