Requirements

Bachelor’s degree from a college or university accredited by an agency recognized by the U.S. Department of Education in social work, Human Services, Psychology, or a related field AND two years of paid, on-the-job social service experience.
OR A general bachelor's degree from a college or university accredited by an agency recognized by the U.S. Department of Education AND four years of paid, on-the-job social service experience.
Proficiency with state and federal program regulations affecting adults with functional impairments.
Skilled in using assessment tools, care planning software, and maintaining accurate client documentation.
Knowledge of local social services, healthcare systems, and community resources.
Expertise in crisis intervention, discharge planning, and caregiver support strategies.
Strong interpersonal skills to engage with clients, families, and community partners.
Excellent written and verbal communication for assessments, reports, and community presentations.
Must meet the driving requirements of this position and comply with the Kitsap County Vehicle Use Policy.
Must obtain a valid Washington State Driver's License prior to hire and maintain a satisfactory driving record as outlined in the Kitsap County Vehicle Use Policy.
Will or may have unsupervised access to children under 18 years of age, a vulnerable adult, or a vulnerable person, or access to a secured/confidential facility or computer system(s). Extensive criminal background check will be conducted.

Responsibilities

Conduct Comprehensive Assessments: Lead detailed interviews and evaluations using standardized state tools to determine clients’ functional needs and eligibility for services. Assess formal and informal support networks and guide clients toward the appropriate care level.
Develop and Implement Service Plans: Create individualized care plans, authorize services, and coordinate implementation with clients, families, health professionals, and service agencies to ensure effective outcomes.
Provide Resource Information and Referrals: Act as a knowledgeable resource for clients and caregivers, deliver community presentations, and refer to providers and interdisciplinary teams to ensure comprehensive care. Stay current on local and emerging services.
Maintain Client Documentation: Develop and maintain complete client files, documenting assessments, service authorizations, and follow-up actions. Ensure timely receipt of signed forms and applications to maintain program eligibility. Stay current on program and policy changes.
Perform Crisis Intervention: Provide supportive counseling and guidance to clients and caregivers who are vulnerable, functionally impaired, or unable to care for themselves.
Represent the Division Professionally: Serve as an ambassador for the department, providing outreach, education, and marketing services to the public as needed.
