About The Position

We’re looking for a dedicated and compassionate Medicaid Care Coordinator (MCC) to join our team! In this role, you’ll be essential in coordinating Medicaid application processes, bridging cultural and linguistic gaps, and ensuring our patients receive the healthcare access they deserve. You’ll serve as a liaison with state agencies, community organizations, and our clinic staff to promote health equity and increase Medicaid eligibility for our patient population.

Requirements

  • High school diploma or GED
  • Experience working with low-income or underserved populations preferred.
  • Strong customer service, problem-solving, and interpersonal skills.
  • Ability to communicate effectively, both orally and in writing.
  • Proficiency in Microsoft Office and report generation.
  • Knowledge of community resources and healthcare systems.
  • Tact, patience, and cultural competency in working with diverse populations.
  • Valid driver’s license, reliable transportation, and proof of insurance.
  • Availability for occasional evening/weekend outreach events.
  • Ability to travel between clinic sites and to meetings in Columbia or Jefferson City, MO.
  • Ability to complete mandatory CMS Certified Application Counselor (CAC) training and exam, a State of Missouri Navigator License, and ongoing continuing education as required.

Responsibilities

  • Coordinate and manage the submission of MO HealthNet and Presumptive Eligibility (PE)/Temp Medicaid for Pregnant Women (MPW) applications to the Missouri Family Support Division (FSD).
  • Serve as a liaison with the FSD and Missouri Primary Care Association (MPCA) to train staff and volunteers on MO HealthNet application processes.
  • Bridge cultural and linguistic needs to eliminate healthcare barriers for Medicaid patients in our downtown clinic lobby.
  • Promote Health Center services and programs at community events and special projects to expand Medicaid eligibility.
  • Ensure PE/Temp MPW applications are reviewed for accuracy and submitted daily.
  • Track and report all MO HealthNet applications submitted to the FSD on a monthly basis.
  • Train new staff on Medicaid application procedures and workflows.
  • Conduct “in-reach” with uninsured patients and outreach to non-patients to educate and enroll individuals in MO HealthNet/CHIP or the Health Insurance Marketplace.
  • Identify community resources and build partnerships to connect consumers with healthcare options.
  • Travel to various clinic locations and off-site events to achieve outreach goals.
  • Maintain confidentiality in compliance with HIPAA and organizational policies.
