Requirements

  • Licensed Practical Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Current CPR certification.
  • Interest in gerontological nursing, rehabilitative and restorative clinical practices.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Responsibilities

  • The purpose of your position is to put “Smiles in the Eyes of our Residents” by fulfilling Presbyterian Homes’ mission, purpose, motto, commitment to service, promise, and practices.
  • You will be responsible for the provision and monitoring of high-quality clinical care and services to residents consistent with regulations and established clinical best practices.
  • You will monitor the work of Medication Assistants, Resident Assistants and Resident Aides in the community on his/her assigned shift.
  • You will report directly to the RN, Clinical Coordinator or Clinical Administrator as designated at the site and may take direction from the Community Coordinator.
  • Oversee and coordinate all aspects of medication administration for Assisted Living residents in accordance with physician orders, state regulations, and community policies.
  • Monitor resident medication profiles for accuracy, effectiveness, and potential concerns, promptly addressing discrepancies or changes in condition.
  • Maintain appropriate medication inventory levels, ensuring timely ordering, storage, and disposal in compliance with regulatory standards.
  • Conduct routine audits of medication records, administration practices, and documentation to ensure adherence to compliance and quality standards.
  • Provide guidance, oversight, and support to caregiving staff responsible for medication administration, including training, competency validation, and performance feedback.
  • Serve as a primary liaison between the community, healthcare providers, and pharmacies to coordinate medication orders, refills, clarifications, and changes in treatment plans.
  • Monitor for and report adverse drug reactions, medication errors, and safety concerns, implementing corrective actions as needed.
  • Support resident care planning by collaborating with interdisciplinary team members and contributing clinical insights related to medication management.
  • Ensure all documentation related to medication administration, changes, and communications is timely, accurate, and compliant with regulatory requirements.
  • Promote a culture of safety, accountability, and resident-centered care.
  • Stay current with best practices, regulatory guidelines, and community protocols related to medication management.
  • Support quality improvement initiatives and participate in surveys or inspections as needed.
