About The Position

CLASS is hiring a MED Coach. The MED Coach advocates for persons with disabilities to become as independent as possible in integrated employment and home settings in the community. Daily, the successful MED Coach monitors the health, well-being, and safety of clients. The MED Coach acts as a role model, provides training and support to assist clients to be successful in the lifestyle of their choice. The MED Coach will be cross trained to work in either employment or home sites with one or more clients. As part of a Service Action Team (SAT), the MED Coach practices teamwork with co-workers and team members from other county SAT’s and throughout the organization. The MED Coach assists in the development of volunteers and natural support to decrease clients’ dependency on paid staff and administers/monitors medications for clients following CLASS LTD policies and standards of care. The MED Coach-coordinates physician ordered prescription and over the counter medications for clients as delegated by Health Service Team and/or department leads; documents Medication Administration Records System (MARS) logs for medication administering, changes per physician’s orders reflecting changes in dosage, new medications and discontinued medication/s. Prior knowledge of medications and reactions desired. PC literacy with Microsoft office is preferred. Must be detail oriented. The Direct Support Professional is a MED Coach who has successfully completed the required training(s). The ability to administer medications to clients through competency-based training and nursing delegation are conditions of employment at CLASS LTD for all Coaches.

Requirements

  • Must be 18 years of age or older.
  • Must have reading, writing and math skills at an acceptable level.
  • Proficiency in meeting documentation and record keeping requirements.
  • Must be able to stand for extended periods of time, bend, crawl, stoop, and walk.
  • Be able to lift a minimum of 50 lbs., when necessary.
  • Maintain a current and valid driver's license of the state of residence and be determined as an "insurable driver" by the automobile insurance carrier for CLASS LTD.
  • Ability to operate Braun lift (van and to maneuver wheelchairs in a van and place in wheel-locks, when necessary).
  • Must be able to work with male and female clients.
  • Ability to communicate effectively with others.
  • Read, comprehend & compose written material as required for case notes, plans, and policies.
  • Work as an effective team member and facilitator of the team(s) to which assigned.
  • Attain minimum required in-service training each year.

Nice To Haves

  • Prior knowledge of medications and reactions desired.
  • PC literacy with Microsoft office is preferred.
  • Must be detail oriented.
  • Completed an additional 6 hours of online med dispensing related training within 3 months of completing med training.
  • May require a commercial driver’s license (CDL) for some positions (most coaches do not need to have a CDL).

Responsibilities

  • Monitors the health, well-being, and safety of clients.
  • Acts as a role model, provides training and support to assist clients.

  • Administers/monitors medications for clients following CLASS LTD policies and standards of care.
  • Coordinates physician ordered prescription and over the counter medications for clients as delegated by Health Service Team and/or department leads.
  • Documents Medication Administration Records System (MARS) logs for medication administering, changes per physician’s orders reflecting changes in dosage, new medications and discontinued medication/s.
  • Meet MAR requirements by reviewing and updating medication schedules accurately and in a timely manner.
  • Weekly med counts at all CLASS locations within your assigned work locations.
  • Dispense client medications as outlined in med training, policy and procedures determined by CLASS LTD.
  • Comply with all federal, state and local laws regarding communicable and infectious diseases and CLASS LTD policies rules and practices regarding immunizations.
  • Maintain a clean med dispensing area, and related supplies and other related equipment.
  • Math calculations needed to complete support log totals.
  • Communicate effectively with others.
  • Read, comprehend & compose written material as required for case notes, plans, and policies.
  • Work as an effective team member and facilitator of the team(s) to which assigned.
  • Attain minimum required in-service training each year.
