Requirements

• Minimum of 2 years of experience in MDS

Responsibilities

• Conduct thorough assessments of residents' needs and develop individualized care plans in collaboration with the interdisciplinary team
• Evaluate and revise care plans as necessary, ensuring alignment with regulatory requirements and individual resident needs
• Coordinate and facilitate care planning meetings with the interdisciplinary team, including family members and caregivers
• Develop and maintain accurate and detailed medical records, ensuring compliance with regulatory standards
• Provide guidance and support to nursing staff on resident care and care planning
• Collaborate with other departments, such as social services and therapy, to ensure comprehensive care
