Your expertise will play a crucial role in ensuring the highest quality of care for our residents. This is a dual role as your focus will be working closely with our current MDS coordinator and our Social Services Director. Position Overview: In this role you will be assisting in the following: Plan, organize, implement, and evaluate a comprehensive Social Services/MDS program Collaborate with the interdisciplinary team to promote resident rights and psychological well-being Prevent, identify, and report resident abuse, neglect, or exploitation in accordance with law and licensure Complete psycho social assessments, social histories, and applicable MDS sections to guide care planning Maintain accurate and timely documentation in compliance with federal, state, and company requirements Participate in the development and implementation of interdisciplinary care plans Provide therapeutic interventions to assist residents with transitions and adjustment to long-term care Educate residents and families on advance directives, resident rights, and available community resources Coordinate discharge planning, including referrals for home care and post-discharge services Initiate and participate in Discharge Transition Plans and discharge education Maintain relationships with community agencies and referral partners Participate in quality assurance and interdisciplinary team meetings Educate staff on psycho social needs, cultural diversity, resident rights, and abuse prevention Collaborate with nursing leadership and administration during inspections and surveys Provide clinical support during crisis or catastrophic events Perform other duties as assigned to support resident well-being and facility operations Works in collaboration with the Interdisciplinary Team to assess the needs of the residents; provides an interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. Assist with coordination and management of the daily stand-up meeting, including review of resident care and the setting of the assessment reference date(s). Monitors MDS and care plan documentation for all residents; ensure documentation is present in the medical record to support MDS coding. Assist DON with the identification of a significant change, physician orders, and verbal reports to ensure that the MDS and care plan reflect those changes. Assures the completion and timeliness of the RAI Process from the MDS through the completion of the plan of care. Reports trends from completed audits to the Quality Assurance Committee. Maintains confidentiality of necessary information. Other duties, responsibilities, and activities may change or be assigned at any time.
