About The Position

Eliza Jennings Home on the Westside of Cleveland is hiring for a full-time MDS Coordinator. Eliza Jennings Home has a census capacity of 126 residents. We have (2) full-time MDS coordinators to assist the needs of the residents. The MDS Coordinator requires 40/hours week, but has the ability to have a flexible work schedule. Willing to train the right candidate.

Requirements

A current valid RN nursing license issued in the state where the person will practice from an accredited school of nursing.
Two years of MDS experience in the long-term care setting.

Responsibilities

Oversee, coordinate and conduct comprehensive resident assessments to comply with regulations, maximize reimbursement, and reflect quality measure/indicator outcomes.
Accurate documentation of all services in order to assure accurate reimbursement from all payor sources.
Coordinate the development of a written care plan and assessment for each resident and review and revise as appropriate.
Work with established departmental, center and corporate policies and procedures, objectives, quality improvement program, and safety environment.
Develop, update, and supervise implementation of interdisciplinary team care plans.
Coordinate and oversee unit compliance with comprehensive assessment requirements.
Complete MDS assessments according to required guidelines.
Review assessments for consistency and completeness.
Submit MDS assessment, maintain files and submission reports.
