About The Position

Saratoga Medical is hiring one MDS/RAI Nurse for an onsite contract opportunity at a long-term care facility serving veterans in Bemidji, Minnesota. The role coordinates resident assessments and care planning. The contract may be extended. The schedule is 40 hours per week, onsite five days per week.

Requirements

• Current, unrestricted Minnesota RN license.
• At least 2 years of MDS/RAI coordination experience in an SNF/LTC setting.
• Knowledge of MDS 3.0, the CMS RAI Manual, and EHR/MDS software.
• Ability to pass the Minnesota DHS background study and criminal background check.
• Current CPR/BLS certification.
• TB screening and immunizations.

Responsibilities

• Manage MDS assessments and the assessment calendar.
• Submit assessments through iQIES and correct errors.
• Coordinate interdisciplinary care plans and care conferences.
• Manage quality measures and PDPM/case-mix requirements.
• Provide staff education.
